The success of implant treatment does not depend solely on the implant and prosthetic restoration itself.
Equally important - and often underestimated - are the tissues of the The soft tissue surrounding the implant, i.e. the gingiva. Why does the gum around the implant matter?
The natural tooth is surrounded by a fibrous ligamentous apparatus and attached gingiva, which protect it from mechanical factors and bacteria.
The implant - as an artificial component - does not have these natural ligaments.
Therefore, the gum around the implant must be:
- stable,
- thick,
- permanently attached to the bone substrate,
- free from tension of the mobile mucosa.
If there is a movable mucous membrane around the implant, a so-called 'tissue pulling' effect can occur, which in the long term leads to:
- gingival recession,
- exposure of the implant surface,
- adjacent bone atrophy,
- tissue inflammations perinatal (periimplantitis),
- aesthetic deterioration - particularly in the anterior segment.
Where are soft tissue procedures most commonly performed?
1. Aesthetic zone (anterior teeth)
- Here, every millimetre of gum has visual significance,
- There needs to be adequate gingival thickness, symmetry and full coverage of the titanium or zirconia structure,
- Often grafts are performed to thicken the biotype and achieve a natural gingival line.
2. lateral zones (molars)
- They require functional stabilisation of the gingiva,
- The idea is to keep the implant shielded and insensitive to the micromovements and chewing power,
- Treatments in this area are of biological and protective importance, even if aesthetics are not key.
What treatments are performed at Unident Union®?
1. Connective tissue graft from the palate
- A thin section of connective tissue is taken from the palate,
- Implanted in the vicinity of the implant to thicken the gingiva,
- Increases tissue volume, improves aesthetics and tightness,
- Used mainly in the aesthetic zone, but also in lateral segments where the thickness of the gingiva is too small.
2. Adherent gingival graft (free gingival graft)
- The aim is to replenish the attached gingiva,
- It stabilises the tissues around the implant and protects against bacterial penetration and inflammation,
- The graft usually comes from the palatal area,
- Particularly important in areas where there is no natural attached gingiva and only mobile mucosa is present.
3. procedures to thicken the gingival biotype
- A thin biotype (clear, delicate gums) increases the risk of recession,
- The treatment consists of adding a layer of tissue submucosally, no notches,
- The result: more solid, resistant gums.
4 Tunneling techniques, envelope and collagen matrices
- Performed in situations where a palatal graft cannot or will not be taken,
- Alternatively, biomaterials can achieve a similar volumetric effect,
- Modern and comfortable methods, used in selected cases.
Why are soft tissue treatments so important?
- They protect the implant from micromotions mucous membranes,
- They protect against infection,
- Improve long-term biological stability,
- They enhance the aesthetics of the smile - especially when zirconia implants are used and natural coverage is needed,
- Facilitate hygiene and self-cleaning of the area cervical,
- Minimise the risk of inflammation of the tissues around the implant (periimplantitis).
When do we perform soft tissue augmentation?
- Prior to implant placement - if gingival conditions are unfavourable,
- During implantation - we often combine procedures in one visit,
- After implant integration - when there are signs of recession, asymmetry or tissue deficiency.
