Dead teeth - removal or treatment?

At Unident Union®, we emphasise root canal treatment as the method of choice. The alternative to keeping a dead tooth in the mouth is its removal and immediate implantation. We make the decision together with the patient based on medical knowledge, individual conditions in the mouth and the patient's preferences.


A healthy tooth is a living organ, fully integrated into the body. It is made up of three tissues: enamel, dentin and pulp. The enamel is a hard, mineralised protective layer, while the dentin - the main building block of the tooth - is highly innervated and rich in dentinal tubules, through which defence and regeneration processes take place. A key role is played by the pulp, which contains the blood vessels that nourish the tooth, as well as lymphatic vessels and nerves.

When the balance of processes in a tooth is disturbed due to deep decay, trauma or mechanical damage, the pulp becomes inflamed and then necrotic. In this situation, the standard solution is root canal treatment under the microscope, which involves mechanical removal of the pulp, professional cleaning, disinfection and tight filling of the canals. 


This procedure eliminates pain and preserves the hard tissues of the tooth for years. Even if the crown of such a tooth is severely damaged, we can reinforce it with an onlay or prosthetic crown and then regularly check its condition. A root canal-treated tooth is able to survive in the mouth for many years without causing problems. However, we realise that this is the method of choice and not every patient wants to opt for it. Below, we explain why.

Why can dead teeth be a problem?

Root canal treatment does not completely remove the risk of infection. Due to the complex structure of dentin, anaerobic bacteria can remain in the microscopic tubules and produce toxins. There is a risk that such compounds can interfere with the body's enzymatic functions, affect the immune system and lead to chronic inflammation, even in distant organs.

In addition, chronic inflammation, invisible to the naked eye, can develop around the root of a dead tooth. At Unident Union®, we therefore recommend regular diagnostics (CBCT, X-ray) of root canal treated teeth to prevent such consequences. However, we are unable to force patients to make follow-up appointments, and untreated inflammation can cause localised bone atrophy and lead to periapical osteitis. In such cases, the body has no effective way of combating the infection, which can lead to it spreading around the periphery via a network of blood vessels.

What is the procedure?

  1. Diagnostics - we perform imaging studies (X-ray/CBCT tomography) and assess the bone condition and conditions for immediate implantation.
  2. Extraction of a tooth - We carry out the procedure atraumatically, taking care to protect the surrounding tissues.
  3. Preparation of the implant site - we use techniques to aid bone and gum regeneration, including PRF membranes rich in growth factors.
  4. Immediate implantation - We place an implant in place of an extracted tooth, which prevents bone atrophy and minimises the number of procedures.
  5. Healing process and crown restoration - After the period of integration of the implant into the bone, an aesthetically pleasing and functional ceramic crown is made.
