What is root canal treatment and when is it indicated?
Root canal treatment (endodontic) is a procedure to remove infected or dead pulp and to clean and fill the root canal system. Indications can range from acute pulp inflammation with accompanying pain to cases of intravital treatment - e.g. after deep preparation of the tooth during preparation for the under prosthetic crown.
In many cases, root canal treatment is an effective way to preserve your own tooth, under the condition That it will be carried out in accordance with current knowledge and with an appropriate biological regime.
Treatment regime - root canal therapy without compromise
W Unident Union® root canal treatment always follows a full technological and microbiological regime, which significantly increases the effectiveness and safety of the procedure. It includes:
- Cofferdam - a flexible membrane that insulates the tooth from the oral environment, prevents re-infection of the canals with saliva or bacteria.
- Treatment microscope - allows the entire root canal system to be precisely found, prepared and cleaned, including invisible branches.
- Lubricants and ultrasound-activated rinses - for dissolving residual pulp, bacteria and biofilm and reaching the microleakages.
- Ozone therapy - as a complement to classical disinfection. Thanks to its bactericidal and virucidal properties, ozone makes it possible to reduce the presence of microorganisms and toxins (e.g. LPS) even in hard-to-reach areas, without risking the surrounding tissues.
With this regimen, we can significantly reduce the risk of infection recurrence and thus increase the long-term success of treatment.
When do we recommend root canal treatment and when do we analyse the case individually?
W Unident Union® root canal treatment is recommended in many cases - also in situations of intravitreal treatment of a tooth where extensive infection has not yet occurred. Such treatments, especially with adequate isolation and sterile conditions, do not carry systemic risks, so the decision to carry them out usually does not require extended diagnostics.
In contrast, we take a more cautious approach to cases where:
- there has been prolonged inflammation in the apex of the tooth,
- The patient reports systemic symptoms, the cause of which may lie in the tooth,
- general illnesses are present (e.g. Hashimoto's, other autoimmune diseases, immune disorders),
- The patient has previously had poor root canal treatment and is considering reendo,
- there are suspicions of infectious outbreaks that could affect other organs.
In such cases, we recommend extended diagnostics, including:
- imaging studies (CBCT),
- blood tests (e.g. LPS, cytokines, PEROX, CRP),
- in selected cases, scintigraphy leukocyte,
- and, of course, a detailed medical history.
However, this does not mean that these examinations are always required - they are a recommendation in selected clinical situations, allowing informed decisions to be made with the patient's general health in mind.
Root canal treatment or extraction and implant?
In selected situations - particularly where there is a real risk of the inflammation spreading to other body systems - we also consider alternative management together with the patient, such as:
- Removing a tooth and replacing it with an implant,
- prosthetic restoration on ceramic implant,
- carrying out reendo only after the inflammation has calmed down beforehand.
We never make a decision hastily - our aim is to find the most biologically safe solution, but also one that is comfortable and acceptable to the patient.
